Community Care Access is an integrated system of health and personal
support services that are arranged for clients based on assessed
need. It provides telephone screening and needs assessment 24 hours
a day, 7 days a week. Assessment and admission for all Facility
Living options, including continuing care, is arranged through Community
Care Access at 496-1300.
Community
Care Access coordinates all continuing care placements for the cities
of Edmonton, Fort Saskatchewan, Leduc, Spruce Grove and St. Albert,
and the counties of Leduc, Parkland, Strathcona and Sturgeon (and
communities within their geographical areas), as well as the Town
of Devon and communities in the eastern part of Yellowhead County.
Community Care Access also arranges to provide ongoing support and
assistance to individuals who may not be suitable for referral to
a specific program but who do require some case management until
their needs are resolved.
Community
Care Access is operated by Capital Health, one of nine regional
health authorities in Alberta, that serves a population of one million
residents in the region. Capital Health was formed in June 1994
during the process of health reform and regionalization initiated
by the Alberta Government. Capital Health coordinates continuing
care services with public, voluntary and private partners. It is
the largest employer in the region, including contract and funded
affiliates such as CapitalCare.
