The
Young Day Adult Day Support Program
enables participants to live at home and come to the centre on a
regular basis during the day for social and rehabilitative activities.
The Dickinsfield program has been operating since January, 1993.
Care
Team
The
care
team meets regularly to identify
and address the needs of all program clients through team meetings
and ongoing interdisciplinary consultation.

Eligibility
Young adults who:
-
are 18 to 55 years of age.
-
can benefit from a structured program with rehabilitation
-
have potential and/or nursing care or management needs
Hours
of Operation
Monday - Friday 9:00 a.m. - 5:00 p.m.
Fees
The fee is $15 per visit. Transportation is available through Disabled
Adult Transportation Service (DATS). Participants are responsible for DATS
costs. Families are encouraged to provide transportation when possible.
