Care
Team
CapitalCare staff work as a friendly, supportive team to create a
care plan that meets residents’ individual needs, adapting the plan
as needs change. The resident and family are key members of the care
team.
Services
Nursing - Nursing
staff provide personal and medical care to residents. Each unit has
a care manager who coordinates the residents’ care planning and
discusses any concerns.
Medical –
Physicians who care for CapitalCare residents are members of the
Capital Region Medical Staff. Physicians will consult with the
resident and family about health concerns.
Pharmacy –
Medications that are ordered by the residents’ physician are
provided by the CapitalCare Pharmacy Department. Centre pharmacists
keep ongoing, up-to-date records of medications, medical conditions
and allergies to monitor residents’ health and safety.
Recreation Therapy - The recreation therapist helps residents maintain their hobbies
while encouraging new leisure activities.
Occupational Therapy - The occupational therapist assists and teaches clients to do as
much for themselves as possible. This may include bathing, dressing,
eating or communicating.
Physical Therapy - The physical therapist assists with development of strength,
balance, coordination and mobility.
Social Work -
The social worker is available to provide counseling, resource
information, referral and advocacy on personal matters.
Dietary
– The dietitian takes into account
residents’ food likes and dislikes, as well as any religious and
cultural preferences.
