The Community Day Support Program
provides medical, nursing and rehabilitative services and
assistance with personal care for adults living in the community.
Care
Team
The
care
team meets regularly to identify
and address the needs of all program clients through team meetings
and ongoing interdisciplinary consultation.

Eligibility
The Community Day Support Program was designed for clients who:
-
live
in the community or are ready for discharge to the community
-
require assistance to meet health needs or improve health
condition
-
need
to retain their own physician
-
have
care needs that can be met by available staff at the centre
-
are
able to participate in group activities
-
may
benefit from at least two of the services offered.
Hours
of Operation
Monday - Friday 9:00am - 3:00pm
Fees
The fee is $15 per visit. Transportation costs are the
responsibility of clients and families. Program staff will assist
you to arrange transportation to and from the program.
