The
Sub-acute Care Programs operated by CapitalCare are located at Norwood and Grandview. The programs
are for people who have been in hospital and need some extra time
and care before returning home. The programs provide 24-hour care
in a centre on a short-term basis.
The medical sub-acute care program is provided at
Norwood.
The program offers care for patients with significant, but stable
medical conditions who require a period of medical intervention
or post surgical convalescence. The care is provided in a less costly
setting following the acute phase of an illness or injury. The goal
of the program is to return the patient to the community.
The orthopedic sub-acute program is located at
Grandview.
It provides in-patient care for people who are recovering from orthopedic
surgery or bone injury and need time in a hospital to regain their
strength before returning home. Our goal is to ensure the patient
is able to manage safely at home and to prevent hospital re-admission.
We offer a homelike setting, which we believe helps patients feel
more comfortable and at ease while taking part in our program.
The two sub-acute programs opened on July 1, 1995.

Planning
Discharge planning begins on the day of admission. Discharge dates
are determined by the care
team
with input from the patient.
