The Transition Unit developed from a partnership with Capital Health's
acute care hospitals, CapitalCare and Capital Health Regional Continuing
Care Services. The unit opened April 10, 1997, in the Angus McGugan
Pavilion at Norwood.
The program is designed for patients with complex medical conditions
who no longer need treatment in an acute care hospital but it is
not clear whether they should be discharged home or to continuing
care. The admission to the Transition Unit allows time to fully
assess the individual's long term needs and discharge destination.
Care
Team
The
care
team meets regularly to identify
and address the needs of all program residents through team meetings
and ongoing interdisciplinary consultation.
Eligibility
The Transition Unit is suitable for the following patients:

A patient in an acute care hospital is recovering from an illness
and is moving to another level of care or returning home, if able.

A continuing care centre resident is in an acute care hospital
and needs to be transferred to a different continuing care centre
because of special needs.
Fees
If a patient is approved for a continuing care centre, a daily charge
comes into effect.
